Our Commitment To Data Security
The security of your Personal Information is important to us. We use standard security protocols and up to 256 bit encryption in our efforts to maintain the security of the Site and the Site is secured by thawte® using Secure Socket Layer (SSL) cryptographic protocols. We have measures in place to protect your Personal Information once it reaches us. Transmissions over the Internet are never 100% secure or error-free. As a result, we do not, and cannot, absolutely guarantee the security, accuracy, or completeness of Personal Information.
Types of Information We Collect and How We Use It
This section of our Policy describes in detail the type of information that we collect. The Personal Information collected through the Site is voluntarily submitted by you or by your physician. Other than to your physician (as described below), we do not sell or otherwise disclose to third parties any Personal Information provided by you without your permission. We may disclose Personal Information as necessary or appropriate to respond to subpoenas, judicial processes, or legitimate requests by law enforcement officials; if we believe in good faith that the law requires it; where we believe in good faith that it is in the interest of the health, safety, or protection of any third party; or to protect ourselves and our respective affiliated companies or partners against liability.
FootSourceMD Account Information. If you have been sent an email from your physician after executing an authorization form at your physician's practice, you may click on the link in the email to establish a user account. This will allow you to receive personalized product recommendations related to your condition, and to store these product recommendations in your account. Your Site purchase information may be shared with your physician.
You may also choose to sign up for a user account without having signed an authorization form or having received an account email from your physician. Through the "Log In" link (or, potentially, other related links as the content of the Site changes) on the Site, you may choose to sign up for a FootSourceMD user account. The information we initially collect in the account creation process includes your first and last name, email address, and a password. You may also enter information related to your physician and your physician's practice, but your account information and purchases through the Site will not be shared with or otherwise associated with your physician unless you have created your account through the physician authorization process explained above. We will use the email address you provide to correspond with you regarding account information and specific transactions. We will also use your email address to send you notifications regarding new products and special offers, though you will have the option of opt out of such promotional emails.
You may also purchase products through the Site without first establishing an account. Regardless of your user account status, we do not store credit card information on the Site after purchases are made.
Post Registration Information. After you have registered, your account will contain your name and email address and potentially additional information such as your mailing address, phone number, past orders, saved orders, your shipping preferences, the name of your physician, the name of the physician's practice, past recommendations from the practice, and conditions the physician associated with you. Some of this Personal Information will be supplied by you, and some of this Personal Information may be supplied by your physician who has received your prior express authorization to share the Personal Information. While your listed physician will receive information pertaining to your product orders, FootSourceMD will not sell or otherwise share your name, contact information, or other account information with third parties except only as necessary for completing shipment of orders.
Other Information Collected
Physician Submitted Information and Recommendations.
Partner physicians who have signed up to participate with our online service may present their patients with a hard copy authorization addressing the sharing of certain patient Personal Information with our Site. Personal Information which might be shared with us-and only after the patient has given his or her express written authorization to the physician-includes patient names, email addresses, conditions, and product recommendations. If you have given your physician permission to share this information, you may receive emails related to this Site and addressing your physician's recommendations with respect to appropriate products for your condition. Even if you have not received a sign up email from your physician, you may also receive promotional emails if you have signed up for a Site user account. If at any time you would like to stop receiving such emails, you may opt out in accordance with the instructions included on the emails. If you would like to revoke the authorization given to your physician with respect to the sharing of your information with this Site, you must do so with your physician at your physician's office.
Third Party Links and Websites.
We may from time to time contract with advertisers or other third parties who place information, advertisements, or other promotional material or links on our Site. If you click on an advertisement or other link provided by a third party and leave our Site, whether through a link or through visiting another website framed within our Site, you are subject to the privacy policies of these advertisers or other third parties. We do not have responsibility for or access to the information collected by these third parties. We will not disclose your medical or health related information to such third parties.
Internet Protocol Address and Aggregate Information.
We may collect and use this information in a variety of ways, including to help analyze Site traffic, understand customer needs and trends, carry out targeted promotional activities, and to improve our services. We do not share this information with third parties. Such information can include, among other things, IP addresses, the search terms you use on our Site, the specific web pages you visit, and the duration of your visits.
Our Site is not intended for use by children under the age of 13, and the features, programs, promotions, and other aspects of our Site requiring the submission of Personal Information are not intended for children under the age of 13. We do not knowingly collect Personal Information from children under the age of 13 without prior parental or guardian consent. If you are a parent or guardian of a child under the age of 13 and believe he or she has disclosed Personal Information to us please contact us as provided in this Policy.
We reserve the right to change our Policy from time to time. Any changes to our Policy will be effective and applicable upon change of our Policy as accessible through the Site. When a change to the Site is made, the date of the new terms will be indicated by a change in the "POSTED" date noted at the top of this Policy. Your continued use of this Site following the posting of any changes to the Policy constitutes acceptance of those changes.
